The Gap Test Is the gap between the door leaf and the frame consistently between 2mm and 4mm? If you can see daylight or fit a 50p coin in the gap, the door may not stop smoke.
The Seal Check Are the intumescent strips (the brushes or plastic strips in the frame) present and undamaged? If they are painted over or missing, the door will fail in a fire.
The Closing Action Open the door halfway and let go. Does it close firmly into the latch without assistance? A door that sits "ajar" is useless in an emergency.
The Hinge Integrity Are the hinges firmly fixed with all screws present? Are there black "leaks" (oil/metal dust) around the knuckles? This suggests the hinge is failing under the door's weight.
The Certification Label Can you see a "Fire Door" label or a color-coded plug on the top or side of the door? If there is no evidence of certification, the door is technically a "Nominal" door and may not be legal for its location.
